Summary
Update of Patient Discharge Tracking List (OPtica) Access patient details using the trust electronic systems and Trakcare Ensure patient flow is maintained and support electronic system update re patients location Monitoring of transport, bookings and ensuring patients are discharged in timely manner, and using appropriate transport methods Liaise with patient flow team regarding expected patient discharges and promoting the use of the Transport Hub. Escalate issues to Clinical Care Coordinators for them to review and aid as required to help facilitate patient flow. Demonstrate well developed written/verbal/listening communication skills Demonstrate good organisational / time management skills and be able work under pressure to meet deadlines. Demonstrate ability to work on own initiative and also to work as part of a team. Demonstrate willingness to be flexible and to work in other clinical areas throughout the trust as and when required in accordance with operational service needs. Demonstrate initiative to respond appropriately to changing situations
