Please see full job description attached. Job Summary To contribute to the delivery of Nutrition and Dietetic service across Liverpool Hospitals NHS Foundation Trust. To assess and treat defined caseload of patients and maintain accurate records. Key Responsibilities 1. To undertake nutritional assessment and calculate nutritional requirements of referred in-patient taking into account medical, social and cultural factors.2. To assist in provision of service to more specialist clinical areas under supervision of specialist dietitian as part of training and CPD.3. To provide nutrition and dietary advice suitable for the patients medical, social and cultural factors which is evidence-based.4. To negotiate change with patients to enable them to achieve dietary targets set.5. To monitor and review nutritional care plans against outcome measures and adjust care plan to facilitate achievement of goals.6. To undertake defined out-patient clinics with support from a supervising Dietitian when needed.7. To assist in developing a range of educational resources which have a clear evidence-base and which are acceptable to users of the service and their carers8. To accurately record all assessments and interventions in medical notes, nursing documentation and dietetic record cards in line with Trust policy. To communicate in writing with other agencies, e.g. GPs, community dietitians about patient care.9. To ensure safe and timely discharge of patients on home enteral feeding liaising with appropriate internal and external agencies.10. To undertake defined patient group sessions, which will facilitate lifestyle changes. This should include planning, delivery and evaluation of the session providing advice to patients which is clear, consistent and evidence-based.11. To assist in delivering formal and informal training to other health care professionals and staff groups, to include pre and post registration training for nurses.12. To advise pharmacy and senior dietitians regarding stock levels of nutritional products at ward level.13. To assist in the clinical supervision of student dietitians including planning of programme during weeks of responsibility.14. To communicate and liaise with other agencies appropriately, e.g. members of the MDT, catering, pharmacy, community dietitians.15. To record and submit clinical and professional activity using manual and electronic means according to departmental standards.16. To attend and contribute to department meetings such as required.17. To contribute to the Trust and Department Clinical Governance agenda by attending Clinical Governance meetings, undertaking audit projects as part of clinical practice and engaging in CPD and the trust appraisal system.18. To contribute to and comment on the department of Departmental Policies and matters relating to nutrition and dietetics and contribute to patient safety through clinical incident reporting. Monitor and maintain health, safety and security of self and others.19. To act in ways which support equality and diversity.
