Responsibilities
Conduct thorough assessments of clients' needs and circumstances
Develop, implement, and review personalized care plans
Provide emotional and practical support to clients and their families
Collaborate with multidisciplinary teams to deliver comprehensive care
Advocate for clients' rights and access to services
Maintain accurate and up-to-date client records and documentation
Conduct home visits and fieldwork to monitor client progress
Provide crisis intervention and support as needed
Stay informed about local resources and community services.
Requirements
Bachelor's degree in Social Work or equivalent qualification
Registered with the relevant professional body
Minimum of 3 years of experience in social work with adults
Strong knowledge of social care legislation and best practices
Excellent communication and interpersonal skills
Ability to work effectively within a multidisciplinary team
Demonstrated skills in assessment and care planning
Strong organizational skills and attention to detail.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £28.90
