Summary
Amdanom ni:
Ym Mwthyn y Ddôl, credwn ei bod yn fraint fawr cael bod yn rhan o fywyd plentyn. Mae ein Gweithwyr Preswyl yn darparu cymorth a gofal cyson am 24 awr y dydd, 7 niwrnod yr wythnos, gan helpu’r plant i fyw bywyd bob dydd a dod i ddeall y byd o’u cwmpas.
Yr hyn y byddwch chi’n ei wneud:
Byddwch yn gweithio fel rhan o’r tîm i sicrhau bod plant a phobl ifanc sy’n cael seibiant byr ac asesiadau yng Nghonwy yn cael y lefel a’r safon briodol o ofal a chymorth. Bydd y gofal a’r cymorth hwnnw’n seiliedig ar helpu plant i fyw eu bywydau’n wahanol i’r ffordd y maen nhw’n ei fyw o ddydd i ddydd, drwy eu cynnwys mewn materion a phenderfyniadau sy’n effeithio arnyn nhw, yn ogystal â helpu plant a theuluoedd i aros gyda’i gilydd.
Fel Dirprwy Reolwr Uned, byddwch yn helpu’r Rheolwr Preswyl i reoli canolfan asesu sydd wedi’i chofrestru ag AGC o ddydd i ddydd, gan arwain gwaith rheoli a datblygu tîm Therapiwtig Amlddisgyblaeth fel rhan o’r ganolfan asesu ac ymyrraeth.
Bydd disgwyl i chi gymryd rôl arweiniol wrth ddatblygu cynlluniau wedi’u haddasu’n bersonol ‘ar gyfer’ a ‘gyda’ plant/pobl ifanc, gan fynd ati i gynnig gofal neu gymorth yn y funud sy’n bodloni’r canlyniadau a nodwyd ar gyfer unigolion ac sy’n dangos parch amlwg at eu hymreolaeth, eu hannibyniaeth, eu gwerthoedd personol a’u hurddas.
Bydd disgwyl i chi ddarparu cymorth i ddatblygu’r gweithlu a chynnig cyfleoedd ymarfer arloesol i fanteisio i’r eithaf ar sgiliau a gwytnwch y staff wrth ddarparu gwasanaeth therapiwtig pwrpasol sy’n galluogi plant a phobl ifanc i wneud synnwyr o’u byd a’u profiadau o ddydd i ddydd.
Profiad ac arbenigedd mewn annog plant i gymryd rhan yn y gwaith o gynllunio eu gofal eu hunain a lle bo’n briodol, cynnwys teuluoedd yn y gwaith o gynllunio gofal ar gyfer eu plant, a meithrin/cynnal perthnasoedd dwy ffordd ar sail ymddiriedaeth, didwylledd, gonestrwydd a pharch o fewn ffiniau’r rôl.
Gwybodaeth ychwanegol:
Rhaid i bob ymgeisydd llwyddiannus gael datgeliad boddhaol gan y Gwasanaeth Datgelu a Gwahardd.
Ymunwch â ni ym Mwthyn y Ddôl i fod yn rhan o dîm sydd wir yn gwneud gwahaniaeth, a manteisio ar y cyfle i ddatblygu eich gyrfa yn y maes gofal preswyl i blant.
Manylion y rheolwr ar gyfer trafodaeth anffurfiol: Nicola Orme, Rhelowr Gweiadur (Nicola.orme@conwy.gov.uk / 01492 574295)
Gofynion y Gymraeg: Mae’r gallu i gyfarthrebu yn y Gymraeg yn ddymunol ar gyfer y swydd hon. Rydym ni wedi ymrwymo i'r Gymraeg ac yn falch iawn o’n diwylliant Cymreig. Rydym yn croesawu ceisiadau yn y Gymraeg a’r Saesneg fel ei gilydd ac ni fydd ffurflenni cais a dderbynnir yn y naill iaith na’r llall yn cael eu trin yn llai ffafriol na’i gilydd. Rydym yn frwdfrydig dros eich cefnogi a’ch annog i ddefnyddio’ch Cymraeg, beth bynnag eich lefel; ac mae gennym ni ddosbarthiadau am ddim ar gyfer pob lefel, wyneb yn wyneb ac ar-lein, i’ch cefnogi chi ymhellach.

You will work as part of the team to ensure that children, young people receiving short breaks and assessments in Conwy receive the appropriate levels and standards of care and support, that is based on helping children to live their life differently as they are living it day to day by involving them in matters and decisions that affect them, as well as supporting children and their families to remain together.

You will be expected to provide workforce development support and innovative practice opportunities to maximise the skills and resilience of the staff, in the delivery of a bespoke therapeutic service enabling children and young people to make sense of their world and their day to day experiences.
