Summary
Job Purpose The post holder will be responsible for the delivery of general practice nursing services to the practice population. Working collaboratively with the general practice team, the nurse will provide a wide range of nursing services, health promotion, and chronic disease management in line with local and national guidelines.Key Responsibilities Deliver evidence-based nursing care to patients in accordance with NMC standards. Undertake chronic disease management including asthma, COPD, diabetes, and hypertension reviews. Provide routine nursing services including: Childhood and adult immunisations and vaccinations Cervical cytology Wound management and dressings Contraceptive advice and support Health checks Provide health promotion, lifestyle advice, and support for self-care management. Support the achievement of QOF and Enhanced Services. Maintain accurate, comprehensive, and timely clinical records. Participate in infection control and vaccination campaigns, including seasonal flu clinics. Work collaboratively with GPs, HCAs, and other allied health professionals to ensure safe and effective patient care. Undertake regular clinical audit, significant event analysis, and contribute to service development. Person SpecificationEssential Registered Nurse with NMC registration. Demonstrable experience in clinical practice. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Commitment to professional development and high standards of care. Desirable Experience in a general practice or primary care setting. Training in chronic disease management (e.g., diabetes, asthma, COPD). Cervical cytology training. Immunisation training (adult and child). Knowledge of the clinical systems EMIS
