Summary
GP Duties and Responsibilities (GP) As part of our GP team, your key responsibilities will include: 1. Patient Care Provide high-quality primary care through face-to-face, telephone and online consultations. Deliver urgent, on-the-day clinical care during duty sessions. Conduct home visits for housebound patients and those in care homes. Undertake proactive frailty management and personalised care planning for elderly and complex patients. Review investigation results, make timely referrals, and manage clinical correspondence and admin effectively. You will not be expected to hold a patient list. 2. Supervision Provide clinical oversight and support to a paramedic within the PCN home visiting team. With prior agreement to participate in the training and supervision of GP registrars. Support professional development, safe practice and effective triage and escalation processes. 3. Population Health Management Participate in initiatives aimed at improving care for frail and elderly patients and reducing avoidable hospital admissions. Work on targeted proactive care initiatives with support from our Care Co-ordinators and Frailty Team. 4. Multidisciplinary Team Collaboration Work closely with team, including pharmacists, MSK FCPs, mental health professionals, care coordinators, GPs, nurses, ANPs, social prescribers and a paramedic. Participate in regular case reviews, MDT meetings, and proactive care planning. 5. Administrative Responsibilities Maintain accurate and timely documentation using EMIS, Docman and ICE. Contribute to achieving PCN and practice performance indicators including QOF, Network DES, and enhanced services. 6. Quality Improvement and Development Engage in quality improvement projects, audits and pathway development. Contribute to a culture of learning and continuous improvement across the PCN.
