Summary

NHS general medical services include: Responding to medical problems presented by registered patients including history taking, examination, investigation, diagnosis, treatment and referral. Working in partnership with patients to provide high quality patient centred care Establishing and following appropriate systems, in conjunction with other staff, to manage common chronic medical conditions within the patient groups specified, including defining the Usual Doctor and creating Care Plans for patients who need them Providing appropriate preventive health care and advice within the context of the needs of the patient group Attending primary health care team meetings and other meetings as may be necessary Undertaking clinical related administrative and non-clinical duties needed for the delivery of medical services Managing patients clinical records to the standards adopted by PfH. Entering data regarding patients and patient care according to practice protocols and participating in audit related activities Providing certification as outlined in current NHS regulations Making home visits to patients as and when clinically necessary Replying to correspondence and writing reports Delivery of OOHSs Delivery of LES/DES/ES that the practice has signed up to. Non-NHS Services The Practice carries out most of the usual additional services for its patients and the postholder will take part in this. Any monies earned in this way are payable to the Practice. Such services include government reports and examinations, insurance reports, legal reports, medical reports, private prescriptions and travel medicine. The post holder is required to keep up to date in medical treatments and procedures. All practice doctors must maintain a Personal Development Plan, which is informed by the Practice Development Plan and reviewed annually. The post holder will be required to take part in 1:1s and appraisal with the Lead GP and Practice Manager. Medical practitioners employed in the Centres are responsible for maintaining the professional integrity and medical practice standards of the practice by: Working within the policies and procedures of the organisation and the practice (including those laid down for prescribing, clinical governance etc.) Participating in the organisations annual appraisal process in addition to the requirement to participate in the NHS Revalidation process for GPs Participating in and contributing to health and social service networks within H&F and service development within those communities Hours of work: The GP led health centres are open from 08.00 hrs to 20.00 hrs 7 days a week. The post holder will be expected to work between 15 and 37.5 hours a week across a 7 day period, and this will be allocated fairly and evenly reflecting the demands of the service. Each doctor is contracted to work a specified number of hours to be rostered by the management of the organisation in consultation with the post holder. All practice GPs are contracted to share responsibility for discharging the terms of the contract by working a proportion of hours outside normal Monday to Friday working hours.
