Summary
To undertake case-finding initiatives to identify patients with Viral Hepatitis and wider liver disease risk factors, working with GP practices, laboratory databases, community organisations, and outreach partners. To liaise with patients to encourage them to enter care and to manage their pathway, ensuring a convenient date, time, and location is agreed. To facilitate the prioritisation of Hepatitis-positive individuals by supporting the coordination of assessments at outreach clinics, Community Liver Health Checks, and ensuring timely linkage into care. To appropriately remind clients of their appointment to facilitate care, which may include sending reminder letters or contacting patients by telephone or text. To support the Viral Hepatitis Team by undertaking blood tests for Viral Hepatiits and performing FibroScan assessments at outpatient clinics, outreach sites, and Community Liver Health Check events across South East London. To motivate and encourage staff within outreach settings (e.g. drug and alcohol services, community venues) to routinely offer HCV and liver health testing using capillary or dried blood spot methods. To monitor the performance of Hepatitis and liver health testing delivered by community organisations such as drug and alcohol services and sexual health clinics. To lead the development and implementation of capillary blood test training within HCV/HBV and liver health outreach clinics, including the development of protocols and supporting materials. To maintain accurate records of training delivered within ODN outreach clinics, monitoring uptake and evaluating the effectiveness of training. To be an effective member of the multidisciplinary team, contributing to departmental objectives and delivery of the Viral Hepatitis CQUIN milestones. To develop and maintain supportive and effective relationships with ODN partners, including community drug and alcohol services, community pharmacists, GP practices, and homeless centres. To collaborate with Viral Hepatitis clinicians and contribute to service improvement and development. To use patient notes, existing databases, and hospital systems to ensure patient data is accurate and up-to-date. To deliver Liver Wellness Clinics across primary care, community, and/or health and justice settings as required. To attend health fairs and community engagement events across South East London to promote liver wellness, deliver FibroScan assessments, and provide Hepatitis testing as required. To support the wider South East London Viral Hepatitis Operational Delivery Network programme team in the implementation of the HCV CQUIN, and contribute to the development and delivery of the ODN across Kings and partner organisations. To identify individuals and groups who would benefit from Hepatitis testing, Community Liver Health Checks, and/or curative treatment within the ODN. To coordinate pathways of care that are tailored and person-centred, ensuring timely access to interventions and appropriate referral to specialist services. To provide clinical interventions such as FibroScan and phlebotomy in hospital, community, and outreach settings. To work across South East London to motivate and empower staff in outreach and community services to embed hepatitis and liver health testing into routine practice.
