Summary
Assist with the management of a team of staff ranging from Health care assistants to Band 5 staff nurses. To assess, plan, implement and evaluate care required for a range of elderly service users. To establish good lines of communication with members of the multi-disciplinary team, we work very closely with Physiotherapy /Occupational therapy/medical social workers and social services and the MAIDT team The post holder must be able to communicate clearly and accurately to other team members on a daily basis. To attend and contribute to multidisciplinary team meetings/ward meetings/clinical governance meetings. To assist in achieving the necessary goals and targets set by the Trust and to ensure quality of care is maintained at all times. To liaise with service users family members/carers, and to offer help and support when required. To assist in ensuring all team members carry out their role in a professional effective manner. To complete audits and give feedback to the team.
