We’re looking for an enthusiastic, HCPC-registered Occupational Therapist to join our friendly multi-disciplinary Reablement Team to embed a therapy led, strengths-based and a holistic approach to short term care delivery. If you’re passionate about helping people regain independence and live safely at home, we’d love to hear from you.

The Role

As a Reablement Occupational Therapist, you’ll play a vital role in helping individuals regain independence and confidence in their daily lives following illness, injury or a decline in function. Working in people’s homes and community settings, you will deliver high-quality, client-centred reablement interventions to a varied caseload – from older adults to those recovering from hospital discharge, surgery, or managing long-term conditions.

You will carry out holistic functional and environmental assessments, set personalised reablement goals, and provide time-limited, targeted interventions that promote self-care and reduce dependence. This includes recommending and prescribing equipment, advising on adaptations and offering practical training to clients, carers and Community Reablement Assistants.

Collaboration is key – you’ll work as part of a multi-disciplinary team alongside Social Workers, Reablement Officers, health professionals and families to ensure clear pathways, SMART goals are met and risks are managed.

What You’ll Bring

  • HCPC registration as an Occupational Therapist.
  • Strong clinical reasoning and problem-solving skills.
  • Experience in community or reablement settings (desirable).
  • Excellent communication and team-working skills.
