Summary


We are looking for an enthusiastic, experienced Band 6 Learning Disabilities specialist nurse to join our busy Community Learning Disabilities Service in Harrow .This is a key role within the team and you will have both generic and specialist responsibilities.

You must have a valid NMC registration with 4 years minimum post qualification experience in a mental health /learning disabilities setting.

Together with other senior colleagues, you will be responsible for the day to day delivery of the service and representing the service as required. This post involves ensuring a high level of specialist provision across the community setting for adults with learning difficulties who have mental health issues and or challenging behaviour.

You will be required to provide supervision to junior staff and mentor students. Under the guidance of the senior nurse and team manager, you will be expected to lead and deliver comprehensive evidence based practice, as well as translating the Trusts’ Research and Development Strategy into local service provision.

Committed to recovery principles, the successful applicant will be self –motivated and able to work to a high standard both autonomously and as part of a team. You will be able to work calmly under pressure, be flexible and responsible to changing needs, able to deliver and support other staff in the safe delivery of services.

The Community LD team is based at Northwick Park Hospital, Watford Road Harrow HA1 3UJ

The nearest station is Northwick Park ,10 minutes (metropolitan line Underground), The post holder will be required to travel within Harrow and to other locations within the Trust.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

The post-holder will be required to work within the multi-disciplinary team structure of the Community Services, providing specialist professional advice and expertise in learning disabilities. The job-holder will be required to carry a caseload of service users within the Community Service. Where appropriate this includes roles of care co-ordinator, placement monitor, care manager and social supervisor. Although based in Harrow the post includes the requirement to manage cases outside the Harrow area.

In addition to having a caseload, the post-holder will be required to adopt an effective and positive team approach and work with the other Care Co-ordinators to ensure effective team delivery of client/ carers’ care. It is expected that the post-holder will make a valuable contribution to multi-disciplinary team assessments, the formulation and implementation of MDT care plans, while working closely with individuals and teams in enabling recovery and independence. He/she will establish and maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
