Summary
Please refer to JD/PS for full details of this role. This role is for a Band 7 Occupational Therapist, who will be based an integrated Reablement Team based in Adult Social Care. The Band 7 Occupational Therapist will provide a high-quality, specialist service to adults and older people with a wide range of disabilities and complex needs, including chronic illness and long-term conditions. This community-based role sits within an integrated multidisciplinary Reablement Team and focuses on promoting independence, preventing hospital admissions, and supporting timely discharges through client-centred, goal-oriented interventions. The post holder will carry out comprehensive assessments, including Care Act Assessments and deliver occupational therapy interventions in clients homes and local communities, working collaboratively with service users, carers, and professionals to develop personalised care plans that support independent living. In addition to clinical responsibilities, the post holder will provide leadership, supervision, and support to junior occupational therapists and Reablement Officers. The role also involves working closely with the OT Clinical Specialist to contribute to service development, training programmes, and clinical pathways, as well as participating in audits, research, and quality improvement initiatives. As part of Role, the occupational therapist will collaborate with physiotherapists, nurses, social workers, GPs, and other professionals to deliver coordinated care that reduces unplanned hospital admissions and facilitates safe, supported discharges. The post holder will be expected to demonstrate strong clinical reasoning, leadership, and communication skills, and to act as a role model for best practice within the team.
