Summary
1. Personal Care Assistance Help patients with activities of daily living (ADLs) such as: Bathing, dressing, grooming, and toileting. Assisting with mobility and positioning in bed or wheelchair. Helping with eating and drinking, ensuring proper nutrition and hydration. 2. Observation and Monitoring Monitor patients physical and mental health, noting any changes in behavior, mood, or physical condition. Report any concerns or abnormalities to the nursing staff or medical team promptly. Keep accurate records of care given, including food and fluid intake, vital signs, and any incidents. 3. Supporting Dementia Patients Needs Use effective communication techniques tailored to dementia patients, such as simple language and non-verbal cues. Provide reassurance and emotional support to reduce anxiety and agitation. Help manage challenging behaviors in a calm and patient manner. Encourage engagement in activities to promote mental stimulation and maintain cognitive function.
