Summary

Ensure that medical staff are made aware of any patient who is unable/unlikely to be able to manage their medicine in safety. Appropriate alternatives should be suggested where necessary. Assist with the development of Patients Own Drugs Schemes and the development of self-medication schemes in line with hospital policy. Provide patient counselling and education to improve concordance with medicines. To develop and maintain stock lists of medications, IV fluids, dressings and miscellaneous products for all wards and ensure appropriate levels of stock are kept (in date) and that these are stored correctly. To perform controlled drug checks to ensure proper recording and balancing of records. To report any inconsistencies or errors found during the controlled drug check to the appropriate line manager. Emphasise the importance of clinical governance, attend meetings when necessary, fill incidents via the Datix reporting system where appropriate. Ensure where possible the therapeutic drug level monitoring of vancomycin and gentamicin and any other drugs with a narrow therapeutic window. Follow up the results with prescribers. Undertake limited training for chemotherapy cover (release checks only) as set out in the internal leave policy. To provide pharmaceutical advice for the dispensing of inpatient and outpatient prescriptions when working in the Dispensary. To participate in any pharmacovigilance activities such as drug recalls. To screen outpatient/inpatient and discharge prescriptions prior to dispensing. Attend monthly Infection Control Committee meetings and undertake any work which these may generate. Act as an antimicrobial steward ensuring adherence to GHA Antibiotic Guidelines and encouraging IVOST where appropriate. Develop and update protocols / formularies / guidelines and policies for the GHA, as necessary. Ensure as closely as possible the continuity of drug treatment for sponsored patients returning from Spain on Spanish drug regimes. Participate when necessary in clinical audits. Participate when relevant in Senior Nursing Professional Development Forum. Work when necessary with the Practice Development Nurse. To dispense drugs, as prescribed, to all GHA clinical areas. Sort and return to shelves, or discard where necessary, any returned medicines from the wards or outpatients. To participate in the Pharmacy on-call rota. To assist with the training of pharmacy staff, nurses, doctors and other healthcare professionals. To work flexibly, responding to the needs of the department, in order to meet service requirements. To provide cover across all aspects of the Pharmacy service, including Pharmacy Stores, as and when required. To participate in and demonstrate a commitment to Continuing Professional Development (CPD) activities. Participate in the department training programme. To undertake any other reasonable duties and responsibilities, as requested by GHA Management.
