Summary

With the Initial Education and Training of Pharmacists reforms and pharmacists being able to prescribe from the point of registration , we have recognised the need to recruit a lead to drive pharmacy clinical transformation and ensure that our interventions result in positive outcomes for patients.

The Senior Pharmacist Clinical Transformation Lead is a part-time role of 22.5 hours a week as either a standalone role or as part of a portfolio.

The successful candidate should be a registered independent prescriber.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

Support and develop advanced pharmacy practice across the whole of the pharmacy service; leading on the delivery, mentorship and development of Pharmacists as independent prescribers and in obtaining recognition as ‘autonomous clinical practitioners.’

Work across the department with pharmacists in primary care roles, domiciliary care roles and inpatient clinical roles.

Work across professional boundaries to promote’ high standards of prescribing and medicines optimisation’’ across all non-medical and medical prescribing professions.

Drive innovation linked to quality improvement. Ensure clinical excellence in the context of productivity and efficiency.

Champion reducing problematic polypharmacy and deprescribing.

Network and work in partnership with key stakeholders in Dorset and nationally in workforce planning and education & training to ensure a ‘fit for purpose’ pharmacy work force which can deliver against the current demand for a ‘highly skilled Pharmacist and pharmacy work force’ and that meets the needs of future trends in healthcare.

Lead on Antimicrobial Stewardship for the Trust.

Support the education and training needs of the pharmacy workforce as they also enhance their practice.

Work as an advanced pharmacist as part of a multi-disciplinary team in a patient-facing role, providing clinical leadership on medicines optimisation and quality improvement.
