Summary
Responsibilities for direct/indirect patient care Provide physical healthcare monitoring and health promotion interventions, implementing care packages under the direction of the Registered Practitioner Report and escalate concerns or abnormal outcomes of monitoring interventions to the Registered Practitioner or Responsible Clinician Monitor and contribute to the review of service user progress where frequent concentration is required against planned objectives and use initiative to act on the results within the sphere of ones own responsibility, including reporting observations and taking action to minimise risks Implement planned care which encourages and empowers service users and carers to maximise self care and autonomy under the direction of the Registered Practitioner Contribute to case conferences and reviews, supporting service users and carers to engage and contribute to their care Engage in the public health, community development and in the reduction of health inequalities Responsibilities for policy and service development implementation Adhere to legislation, policies, procedures and guidelines both local and national. Regularly attend workplace and staff engagement meetings and contribute positively to discussions about the improvement of care Responsibilities for financial and physical resources Exercise personal duty of care in the safe use and storage of equipment Be environmentally aware and prudent in use of resources and energy
