Salary: Band: 4,£25,201p.a. plus a 17 % shift allowance (if applicable)
37 hours, including early, late and weekend shifts
The Service
The Community Rehab Service (CRS) is an integrated health and social care team, supporting adults with a wide range of health and care needs. The service provides a responsive crisis intervention to manage needs at home and prevent unnecessary hospital admissions. It is a city wide service that facilitates timely discharges and provides community base care which is focused no rehabilitation, reablement and recovery in people's homes.
CRS includes Occupational Therapists, Physiotherapists, Therapy Assistants, and Rehab Assistants who work collaboratively to deliver coordinated, person-centred support. The service supports individuals with a wide range of conditions, from simple to complex, aiming to promote independence and improve quality of life.

What is the role?
As an RRA you will provide personal care to individuals in their own home, supporting their dignity and independence in daily living activities such as washing, dressing, toileting and mobility/transfers. The successful candidate will demonstrate a genuine willingness to learn these skills in line with best practice and service standards, with guidance and support from experienced team members.
Following a rehabilitation approach you will help clients to become more independent and enable them to reach their personal goals. With an understanding of and the ability to implement safe moving and handling techniques, you will promote health and safety for clients and their carer givers/family members. You will report promptly and discuss significant changes in clients to appropriate professionals whilst being mindful to any possible safeguarding concerns, reporting accordingly. You will maintain confidential, accurate and timely electronic records whilst working alongside other professionals to help our clients to achieve the best possible outcome. As well ensuring mandatory training is up to date you will attend regular supervision sessions and participate in group supervision.

Who is the person?
As well as being empathetic and compassionate you will demonstrate a clear understanding of the specific needs of the client group, while actively promoting and upholding their rights, independence and informed choices. With literacy and numeracy to enable you to accurately report and record client data, you will have well developed communication skills and the ability to converse at ease with customers in accurate spoken English which is essential for this post. Flexible and adaptable in order to meet the client and service needs, you will be able to maintain confidentiality and be able to work as an effective team member. You must have a full UK driving licence and your own transport as your role will involve visiting service users across the city.
