JOB DETAILS Job Title: Critical Care Outreach Sister/Charge Nurse Reports to: Deputy Matron Critical Care Outreach Team Accountable to: Lead Nurse Deteriorating Patient Band: 7 Unit/Department: Critical Care Outreach Team CSU Directorate: Adult Critical Care Location AfC Job No: 2478c JOB PURPOSE The post holder will work as an accountable, autonomous practitioner to provide comprehensive advanced assessment and care of the deteriorating/acutely ill adult patient leading to early intervention and treatment. The post holder will work in partnership with the Allied Healthcare Professionals Team to initiate appropriate care to prevent further physiological deterioration, identifying and referring those patients who require admission to critical care. The post holder will act as a clinical role model, promoting quality evidence-based practice. Multi-disciplinary team liaison, education and training in the care and management of the deteriorating/acutely ill adult will be key responsibilities. The post holder will hold achievement of the National Outreach Framework Enhanced Competencies and work towards achievement of The National Outreach Framework Advanced competencies identified through annual appraisals. KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Qualifications Registered Nurse (Level 1). Critical Care Course or equivalent competencies Recognised teaching/assessing qualification. National Outreach Framework Enhanced Competencies. Educated to post graduate degree level or equivalent experience. Advanced Life Support provider. Experience Significant post registration experience. Significant Critical Care Outreach experience at sister/charge nurse level. Working with a wide range of professionals including; medical, nursing and management colleagues. Skills Ability to utilise expert/high level communication and interpersonal skills effectively within a multi-disciplinary team. Ability to teach, supervise and assess qualified and unqualified staff. Ability to manage time & prioritise own workload effectively. Proactive towards change and change management. CORE KNOWLEDGE AND UNDERSTANDING An understanding of the organisational structure and working practices of the organisation Understanding of the local and national nursing agenda Awareness of Leeds Teaching Hospitals Trust guidelines and policies PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Clinical Deliver and maintain a high standard of patient focused care in accordance with Leeds Teaching Hospitals Trust guidelines and policies. Work within frequently challenging clinical environments with high levels of physical and mental effort. Have knowledge of pathophysiology, anatomy and physiology in order to assess, plan, implement and evaluate treatment of the critically ill patient. Able to make expert clinical judgements involving complex situations which requires analysis and interpretation to decide on the best course of action/treatment. Competently and confidently care for the deteriorating/acutely ill adult utilising expert critical care skills and knowledge in a variety of clinical settings with indirect supervision as part of the multi-disciplinary team Provide rapid response and support for patients identified by the National Early Warning Score. Undertake a comprehensive advanced patient assessment based on history taking and clinical examination and implement a management/treatment plan. Provide direct patient care and support at the bedside until the patient is stabilised or transferred to an area with a higher level of care. Initiate therapeutic interventions for example to manage haemodynamic and respiratory instability. Refer to other members of the MDT and other specialities as appropriate. Participate in the stabilisation and transfer of critically ill patients to an appropriate environment. Competent in advanced clinical skills such as venepuncture, cannulation, arterial stabs, tracheostomy decannulation, airway management and cardiopulmonary resuscitation. Accurately request pathology and radiological investigations and make clinical decisions based upon the results. Utilise highly developed communication and negotiation skills to deal with challenging/conflict situations ensuring patient centred care. Work autonomously taking responsibility for own decision making, referring to the appropriate physician/ICU Consultant/Outreach Team Leader when advice and further clinical input is needed or when care and treatment goes beyond normal service parameters or scope of professional practice. Carry the crash bleep and respond to 2222 calls. Where required, carry the critical care bed managers bleep out of hours, liaising with the sisters/charge nurses to co-ordinate flow throughout critical care. Respect patients rights to participate in decisions about their care and be able to advise them on complex and personal matters. Communicate highly sensitive information to patients and their relatives, showing empathy and providing psychological support. Maintain up to date clinical records in accordance with the most up-to-date NMC and Trust guidance. Act as a clinical role model, which reflects the philosophy, beliefs and clinical standards of the outreach service. Maintain a safe working environment. Demonstrate an understanding of key infection prevention and control policies including, hand hygiene, MRSA, transfer of patients. Demonstrate individual responsibility for infection prevention and control, challenging non-compliance with policy. Develop and implement evidence based standards, policies and guidelines. Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies. Education and Training Contribute to the promotion of a positive learning environment within the clinical areas across the Trust. Promote evidence based practice wherever possible. Inform, consult and educate members of the multi-professional team regarding the purpose and function of outreach and the function and use of the Early Warning Scoring system. Participate in critical care based education programmes that are set up across the Trust and the University. Lead and participate in identified education/training needs Trust wide for the deteriorating adult. Deliver and participate in education programmes across the trust to enable ward staff to care for the deteriorating /acutely unwell adult. Participate in preceptorship and mentorship programmes. Undertake continuous professional and clinical development, seeking opportunities to enhance skills. Continue to develop specialist advanced clinical skills in response to service need. Maintain annual mandatory training requirements. Keep up-to-date with developments, advancements within critical care by working clinically in the critical care environment.
