Summary
Act as an advocate for both SWFT and GEH on appropriate measures to reduce from tobacco and support/maintain smoke free sites. Act as an accessible professional for the multidisciplinary team (MDT) supporting the management of a caseload for at least 80% of the role as identified in agreed job plan and personal development plan. Provides oversight for unpredictable caseload on an ongoing basis and support TDAs. Autonomously deliver specialist health promotion in line with Trust protocols and National guidance. Provide specialist advice in relation to the care of individual or groups of patients, underpinning theory with specialist training and knowledge. Contributes to the development of the professional strategies, ensuring local action plans are produced, implemented, and evaluated. Maintain and develop the database, digital working, and Greener working practices in line with Trust practice. Represent the Trust at external events, networking with professional colleagues to maximise the opportunities of promoting the Trust and benchmarking practice with peers. Act as a resource providing specialist advice and support across service boundaries to ensure optimum liaison and co-ordination of care. To oversee the Tobacco Dependency Service Advisors to provide delegated caseloads within the speciality and support with their workloads and provide professional oversight to meet patient and service needs. Act as an expert and professional resource to patients, carers, colleagues within the working multidisciplinary team and wider colleagues within the system. Deliver high quality, effective care such as but not limited to:- To provide high quality holistic evidence-based care to patients. To be pro-active in delivering anticipatory and responsive care, promoting independence, healthy lifestyle choices, reduction in health inequalities to improve the overall quality of life in the local population. To provide advice and support to patients using the local and national strategy. To provide patient advice regarding the appropriate assessment according to agreed hospital protocols at SWFT and GEH. Frequent patient expert knowledge in conjunction with health promotion methodology for the services. Recognise and escalate any deterioration in patients condition that requires a change in treatment or re-assessment at the hospital. Frequently assist patients to accept and cope with their condition and provide psychological support: To be able to deal confidently with members of staff and the public where there may be frequent exposure to distressing or emotional circumstances. Exposure to unpleasant working conditions or hazards frequently.
