Summary
The Primary Care Nurse will provide comprehensive nursing care to the practice population, delivering evidence-based treatment for long-term conditions, preventative healthcare, and health promotion initiatives. The role includes supervising and supporting student nurses and working alongside experienced nursing staff to ensure high-quality patient care. Key Responsibilities: Deliver holistic nursing care to patients with long-term conditions (e.g., diabetes, hypertension, COPD, asthma). Provide preventative care, including immunisations, screening, and lifestyle advice. Undertake clinical assessments, develop care plans, and make autonomous clinical decisions where appropriate. Work collaboratively with GPs, pharmacists, and allied health professionals to coordinate patient care. Supervise and mentor student nurses, supporting their learning and professional development. Maintain accurate patient records in line with practice protocols and governance requirements.Contribute to the development and implementation of practice policies and procedures. Contribute to the development and implementation of practice policies and procedures. Participate in clinical audits and quality improvement projects.
