Summary
Our ideal candidate will be / will have: Registered Nurse (RN) or registered Therapists (OT / Physio) with current live PIN on NMC register or HCPC register Recognised teaching/assessing qualification 998/997/mentorship. Continuous Professional Development Portfolio Specialty knowledge and experience in discharge planning and pathways Experience of working within a multiagency health and care system Relevant post registration clinical experience As this role requires travel, a driving license and access to a car is required for this role.
