There is the opportunity for a secondment from Local Authority to this vacancy the secondment would be done on PKC terms and conditions at the grade SM13 (£65,133 - £70,144).

This is an exciting opportunity to lead a programme of transformation of learning disability services in Tayside, breaking new ground in supporting our transition to new models of care and realising our Coming Home ambitions.

You will support the development and delivery of a Whole System Change Programme as it pertains to Learning Disability, coordinating the delivery of a complex programme of work centred around delivering service improvement and commissioning intentions, improving quality through service design/clinical re-design, contracting and performance frameworks. There are three key components of the programme

  1. Return to the community and care planning for all complex Learning Disability in-patients with a view to eliminating delay and meeting the ambitions of the Coming Home Report.
  2. Establish a crisis response team for Tayside to maintain people with complex LD needs in their own families/ homes & accommodation
  3. Together with HSCP leads for learning disability community services develop collaborative commissioning across the 3 x HSCPs.

Working across the three Health & Social Care Partnerships and inpatient learning disability services you will support senior leaders in the delivery of service redesign, efficiencies, and strategic commissioning within a financial framework to deliver a shift in the balance of care and resource from acute care to community and independent living.

Your impact will be demonstrated and measured by a reduction in bed days, reduced inpatient bed base and increased community provision and accommodation for people with Learning Disabilities.

We are looking for someone who will provide expertise, strategic thinking, programme management and leadership in translating relevant national, regional and local policy across health, social care and third sector services. You will be able to develop collaborative working relationships across a complex health and social care landscape with a strong appreciation of the relationship between local and national organisations, providers and commissioners, Local Authorities and other relevant organisations. You will be able to take initiative working alongside Chief Officers and Senior Management Teams, working across organisations and relevant Boards.

Days/hours of work pattern negotiable.

Hours of work are 37 hours per week.

Work location is flexible across various locations in Tayside with line management in Perth & Kinross Health & Social Care Partnership.
