Summary
An exciting opportunity has arisen for a Band 6 Registered Mental Health Nurse to join urgent care services at the Salford Home Based Treatment and Listening Lounge.
Salford Home Based Treatment Team is a 24/7 service that supports individuals in crisis at home, as an alternative to psychiatric inpatient admission. The Listening Lounge in Salford is a 24-hour friendly and supportive community space open to anyone in Salford who has been referred by a health care professional and requires an urgent assessment of their mental health needs without the need for medical treatment.
Our aim is to offer a 1:1 holistic assessment of an individual’s mental health needs from qualified mental health practitioners and support staff in a relaxed and comfortable environment, avoiding long waits in emergency departments.
The service provides brief psychosocial interventions in a calm environment to help identify and promote positive coping strategies, and complete referrals to identified and appropriate services, and signposts to other local support services.
The team operates a 24/7 service, therefore there will be a requirement to cover shift work including nights.
Main duties of the job
-To effectively screen and gatekeep referrals to urgent care services, ensuring that the least restrictive options for treatment are offered
- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.
- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.
- To act as keyworker for service users with support from the multi-disciplinary team.
- Cover the duty / shift coordination role at times.
- Liaison with other agencies involved in a service users care.
- Lead on the discharge or transfer of care for our service users to appropriate agencies.
- To support and supervise junior members of staff.
