Summary

MAIN TASKS AND RESPONSIBILITIES Responsible for Team Seniors and Rehabilitation Support Workers Ensure the team provide a high standard of care for all service users. Contribute to the assessment, development, implementation, and evaluation of programmes of care as part of the interdisciplinary team. To participate in the holistic assessment of service users care and support needs after admission to the service. To act as key worker and advocate for designated service users; liaising closely with other key workers and professionals including report writing, assessments, and recording of information. To participate in planning, implementation, evaluation and reporting of the progress and outcome of service users individual goals and objectives. To provide individualised, evidence based holistic care and approach to service users and their carers. Exercise sound clinical leadership by example and instruction. Competently carry out clinical procedures and use appropriate clinical equipment within scope of practice seeking advice and further development when required Promote effective confidential communication with service users, relatives, families, carers and colleagues (including external personnel) providing information and support to assist decision-making and be willing to advocate on the service users behalf.
