Please note, this a fixed term, 9 month post.

Main duties of the job include the following:
• Conduct comprehensive assessmentsand deliver evidence-based speech, language, communication, and swallowing therapy to adults with neurological conditions in both community and inpatient settings
• Work collaboratively as part of a multidisciplinary team to deliver integrated, person-centred care.
• Develop and implement individualised treatment plansin collaboration with patients, families, and the multidisciplinary team.
• Provide specialist clinical advice and support to patients, families, and carers to enable informed choices and effective involvement in the rehab process.
• Supervise, mentor and support junior staff, assistants, and students to foster learning and maintain high standards of practice.
• Contribute to service development, quality improvement initiatives, and audits to help shape and improve the delivery of the service.
• Maintain accurate, timely clinical records and contribute to care planning, discharge planning, and onward referrals.
• Promote self-management, empowering patients to build confidence and sustain progress beyond formal rehabilitation.
• Participate in regular team meetings, in-service training and CPD, keeping up-to-date with the latest evidence and clinical guidelines in stroke care and community rehabilitation.

Basic Job summary:

· To provide a Speech and Language Therapy service to adults in outpatient, community and inpatient settings who have:

o acquired communication disorders, and/or

o speech and/or

o language disorders, and/or

o eating, drinking and swallowing disorders

· To work independently to a specialist level of expertise and be accountable for own professional actions.

· To work across the inpatient neuro rehabilitation unit and the community stroke team, carrying an independent caseload within each setting.

· To be part of 6 day working rota on the ESD team, with time off in lieu (TOIL).
