About the Team

We’re proud of our recently transformed Older Adult Community Mental Health Team, developed in line with the Community Mental Health Framework. This specialist pathway supports older adults with functional mental health needs and neurodegenerative disorders, where the impact on wellbeing and independence is significant.

Our team is dynamic, multidisciplinary, and deeply collaborative. You’ll work alongside Senior Practitioners from both the Local Authority and the Mental Health NHS Trust, united in our goal to achieve the best outcomes for people in our community.

About the Role

As Team Manager, you’ll lead with empathy and vision, fostering a culture rooted in recovery, strengths-based support, and rights-based practice. You’ll guide your team to deliver flexible, creative interventions that empower individuals to maintain independence and thrive in their communities.

Key responsibilities include:

Providing visible, accessible leadership and real-time performance management.
Driving quality improvement and change management initiatives.
Collaborating across the wider health and social care system to deliver joined-up support.
Inspiring your team to deliver gold-standard care through shared values and professional development.
We’re committed to your growth too – with ongoing supervision, training opportunities, and support from both the Local Authority and AWP.
