The HMR Rapid Response service has an exciting vacancy for a community based band 7 clinical practitioner within the team based at Rochdale Infirmary to support the residents of Heywood, Middleton and Rochdale (HMR).

We are a community based, multi-disciplinary team of skilled professionals, who provide holistic care and support to enable people to remain in their own home and avoid unnecessary admission to hospital.

We strive to provide individualised, person centred care and we put the patient first at all times.

The full time vacancy is an opportunity to join the team made up of advanced practitioners, physio and occupational therapists, pharmacists and community nurses. It is open to appropriately qualified nurses and allied health professionals.

Regular supervision, competency training, performance reviews and Continued Personal Development are actively encouraged.

You will need previous experience within a community team and be ready to take on a new challenge to help shape an evolving service. Rapid Response are a dynamic and forward thinking team that pride themselves in going above and beyond for all.

As a Rapid Response practitioner you will be part of an exciting community-based workforce providing specialised input to meet the needs of the HMR population whilst developing and leading relationships across the rapid response team, preventing unnecessary acute hospital admissions, enabling individuals to remain in their own homes for longer and prevent premature admissions into long term care facilities.

You will participate in the provision of the Hospital@Home service, ensuring that residents of HMR receive timely and appropriate acute care in their own homes.

You will assist individuals and their carers to proactively manage and anticipate their future health needs, support self-care, develop appropriate plans and work collaboratively with the wider multidisciplinary team including, partner organisations such as NWAS, GP / primary care, pharmacy services and wider Integrated Neighbourhood Team members.

You will participate in developing, delivering, and evaluating high quality patient centred services in conjunction with the associated clinical teams to ensure this is incorporated within the Business Group and Trust objectives.
