Summary

You will undertake a variety of duties including surgery consultations, telephone/video consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. You will make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the practice or external organisations. You will assess the health needs of patients with undifferentiated and undiagnosed problems. You will screen patients for disease risk factors and early signs of illness. You will work alongside the Practice and PCN staff to support the achievement of the QOF indicators, ensuring that all data is recorded appropriately. You will work alongside the Practice and PCN staff to support the achievement of the PCN DES requirements, ensuring that all data is recorded appropriately. In consultation with patients and in line with current practice disease management protocols, you will developing care plans for health-related illnesses. You will screen patients for health education. You will refer patients to other care providers as appropriate. You will record clear and contemporaneous consultation notes to agreed standards. You will collect data for audit purposes and contribute to the practice audit programme as required. You will compile and issue computer-generated acute and repeat prescriptions using ESP (avoiding physical or hand-written prescriptions whenever possible). You will prescribe in accordance with the accepted prescribing formulary (or generically) whenever this is clinically appropriate. In general, the you will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care, as set by Royal College of General Practitioners, General Medical council, all other Royal colleges and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.
