Summary

COMMUNICATION Be an active and supportive member of the team, whilst maintaining effective communication with all stakeholders, both internally and externally. Work collaboratively and act as a specialist resource to all stakeholders in both an advisory and directional capacity Maintain first rate comprehensive, succinct and appropriately hand-written and digital records and to communicate information as required with all relevant parties, whilst maintaining confidentiality of the individuals records; conforming to the Data Protection Act and adhering to Caldicott principles. Undertake assessments and reviews across Bury/Stockport and wider areas, where required. Effective co-ordination and management of the assessment and review process Work collaboratively with health and social care professionals in establishing their involvement and perspective on individuals assessed needs as part of the review and assessment process. Gain and impart highly complex and highly sensitive information in situations where there may be barriers to communication, including sensory and cognitive impairment; this may require the use of adapted methods of communication. Fully and actively involve individuals/family/carers throughout the assessment and review process, using effective and timely communication to gain views, communicate outcomes and to arrange care provision. Act as an advocate for individuals who are vulnerable. Providing and receiving complex, sensitive or contentious information, where there are significant barriers to acceptance which need to be overcome using developed interpersonal and communication skills such as would be required when communicating in a hostile, antagonistic or highly emotive atmosphere. Present appropriate cases to Bury/Stockport funding panels Be influential within a complex and challenging environment and maintain the ability to manage challenge and conflict with regard to NHS Continuing Care processes and decision making. INFORMATION RESOURCES, ANALYSIS AND DECISION MAKING The post holder will be required to assimilate highly complex, often contentious and highly sensitive information then translate this into various forms dependant on the recipient to ensure clear but accurate information is shared in a user friendly format. The post holder will be required on a daily basis to analyse highly complex, multi-faceted information to explore different options available, using their judgment in respect of actions required and then negotiate and influence others. PLANNING & ORGANISATION The post holder will be responsible for management of their own caseload and arranging reviews/ co-ordinating the commissioned care package/ reviewing The post holder will organise and coordinate MDT meetings with the professionals involved and patients/ families. PATIENT/CLIENT CARE Responsible for monitoring and ensuring that the standards of nursing care are meeting the expectations of the ICB, in both the hospital, respite provision and specialist placement settings in line with Ofsted and CQC standards. POLICY & SERVICE DEVELOPMENT The post-holder will plan and co-ordinate where required, individualised multi-disciplinary case conferences to clarify contentious issues about clinical care and plan and implement highly complex packages of care. The post- holder will, where required to do so, actively participate in service reviews co-ordinated by other commissioners of providers who are commissioned by GM ICB- Bury/Stockport and its partnership agencies.
