Summary
This strategy includes the following components: Acute frailty services including front door frailty service, rapid access clinics, and ambulatory care, short stay assessment services. Provision of geriatric, medical input to Major trauma patients who are frail with multiple co morbidities. Surgical liaison Service including pre op assessment to optimise medical issues for elective operation patients, peri operative liaison ward round for in-patients on surgical wards. Development of community services including care home input and other support to primary care services e.g. development of virtual ward. Expansion of Falls and Bone Health services including fracture liaison service, joint osteoporosis services, multidisciplinary falls clinic
