About the Role
We're looking for a Care and Support Worker to join our dedicated Reablement Service on a 26.25-hour per week contract. This essential service operates 7 days a week, 52 weeks a year, delivering high-quality care to people in their own homes.
You'll be part of a team that helps individuals regain independence after illness, injury, or hospital discharge, supporting them to live safely and confidently at home.
Working Pattern
4-week rolling rota
AM shifts: 7:00am - 3:00pm
PM shifts: 2:30pm -10:30pm
Includes weekends and bank holidays
Driving licence and access to a vehicle required
About the Service
Our Reablement Service offers short-term, person-centred care to people with disabilities or those recovering from illness. We focus on:
Promoting independence
Relearning daily living skills
Providing dignity and respect
Working collaboratively with professionals and agencies
We also offer:
Bridging care while long-term packages are arranged
Resettlement support to help people adjust after time away from home
Emergency response care during times of urgent need
Training & Qualifications
Essential:
GCSE Grades D$úG in Maths and English
or
Functional Skills Level 1 in Maths and English
Desirable (but not required):
Level 2 in Health & Social Care (or equivalent)
Don't have the Level 2 qualification? No problem!
We'll provide full training and fund your qualification after successful completion of your probation period.
