Summary
Grade: G scp 23 £34,434 to scp 27 £38,220. The post attracts an additional 7% for out of hours working and an essential car user allowance.
Contract: Permanent, 37hrs per week.
Role specifics: Must be 18yrs or older and hold a full UK driving licence.

The Role
An opportunity has arisen for a Community Assessment Officer (CAO) post within the Discharge to Assess function of The Independent Living Service. The Independent Living Service has 26 staff made up of Occupational Therapists, Disability Officer and Community Assessment Officers and includes the Sensory Team and Telecare Team. The aim of the post is to aid with discharge facilitation and community follow up. The post will sit alongside Community Therapy and the Intermediate Tier Social Work team and looks to support people upon discharge who could be at possible risk of readmission to hospital without support. The CAO will be supported to be “trusted assessors” for equipment and have basic moving and handling training to support therapy if required and have access to Reablement if required.
You will be facilitating hospital discharges by supporting individuals who may be at risk of re-admission without appropriate community support, working collaboratively with interdisciplinary teams like Community Therapy and Social Work staff across services to deliver integrated care solutions whilst still working within legislation such as the Mental Capacity Act 2005.
You will be completing Strength Based Care Act 2014 assessments, including support plans and risk assessments, and conducting community follow-ups to ensure service users have the necessary support post-discharge, acting as a “trusted assessor” for equipment provision to promote independence and safety at home.
You’ll be accessing appropriate services, for example Reablement services to support recovery and independence, providing basic moving and handling support to assist therapy interventions when required and contributing to service innovation by helping develop new ways of working across health and social care.
You’ll improve service user and carer experiences through person-centred support and care planning, as well as promoting well-being and independence in all aspects of service delivery. The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.
