Summary

Key Responsibilities: Patient Care and Management: Provide expert nursing care and management for patients with long-term conditions such as diabetes, asthma, COPD, and cardiovascular disease. Conduct thorough health assessments and develop personalised care plans in collaboration with patients and their families. Monitor and review patients' progress, adjusting care plans as necessary to ensure optimal management of their conditions. Health Education and Promotion: Educate patients and their families on managing chronic conditions, including medication management, lifestyle modifications, and self-care techniques. Promote health and wellbeing through patient education sessions, workshops, and community outreach initiatives. Collaboration and Teamwork: Work closely with the multidisciplinary team, including GPs, healthcare assistants, and other allied health professionals, to deliver integrated and coordinated care. Participate in team meetings and case discussions to ensure a holistic approach to patient care. Quality Improvement and Professional Development: Engage in quality improvement initiatives to enhance patient outcomes and service delivery. Stay up-to-date with the latest clinical guidelines and best practices in managing long-term conditions. Participate in continuous professional development opportunities to maintain and enhance clinical skills and knowledge. Clinical Governance and Compliance: Adhere to clinical governance standards and practice policies to ensure safe and effective patient care. Maintain accurate and comprehensive patient records in accordance with legal and regulatory requirements. Qualifications and Experience: Registered Nurse with current NMC registration. Significant experience in managing long-term conditions in a primary care setting. Strong clinical assessment and decision-making skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Commitment to continuous professional development and lifelong learning. Desirable Skills: Experience in conducting chronic disease management clinics. Knowledge of relevant clinical software and electronic health records. Experience in patient education and health promotion activities.
