Summary
This post is unable to support applications from those individuals requiring sponsorship to work in the UK Main job responsibilities: To provide the specialist physiotherapy element in the Lincolnshire Stroke Service, including providing clinical specialist support, professional advice and practical guidance across Acute and Community stroke pathways. To act as case manager to a designated case load which will include ensuring all patients are able to access all elements of their care plan and to ensure the individual care plans are delivered and progressed by the team. To manage a clinical caseload to a high professional standard and provide effective evidence based clinical service for patients as an autonomous practitioner. This will include more advance & detailed assessments with patients having more diverse & complex conditions, providing a diagnosis and develop and deliver an individualised treatment programme to neurological patients, as part of a specialist programme of intervention. To ensure a high standard of care for all patients by effective communication with patients, carers, other team members and the wider system partners.
