Senior Clinical Nurse Specialist - Learning Disabilities

JOB SUMMARY
• To provide leadership, management support and supervision for Community Learning Disability Nurses
• To ensure an effective and quality Community Nursing Service as part of the Community Learning Disability team
• To provide leadership in the development and modernisation of Community Learning Disability Nursing
• To support service delivery to ensure capacity, demand and access to services remains within commissioned targets and national expectation
• To support the development of team functions in relation to Transforming Care, Behaviour Support and Quality Assurance
• To be able to interpret and communicate highly complex information to team members and external parties

Management Responsibilities

To clinically lead the Community Nurses as part of an interdisciplinary team, working within the Community Learning Disabilities Team service policies and procedures.
• To be accountable for Service Level Agreement delegated Activity Targets within the nursing team in order to positively impact on budget performance.
• for the Learning Disability Health Lead with regard to being the designated person in charge of the team, including effective budgetary management of devolved budgets when requested
• To ensure regular Clinical supervision and appraisal, in line with KSF outlines, for all Community Nurses in partnership with the Operational Clinical Lead for Learning Disabilities.
• To ensure effective management of caseloads as part of an interdisciplinary team, including monitoring, recording and processing of referrals
• To ensure robust work plans are in place and are regularly monitored and reviewed via supervision and reflect service requirements
• To develop effective team working as part of the Community Learning Disability Team.
• To support the management of the team in ensuring effective deployment of Community Nursing staff to meet service needs
• In conjunction with the Management Team, respond to and manage complaints as required by in accordance with Trust and Council policy
• To promote interdisciplinary and inter agency working with voluntary and statutory agencies and across professional boundaries.
