We’rebringing together nurses, therapists,healthand therapy assistants to work alongside acute multi-disciplinary teams and transfer of care hubs to promote an efficient, effective hospital-to-home transition for our patients.


Our multi-disciplinary Home First teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.


Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission.


Home First will, whererequired, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

As a Health and Therapy Assistant you will be providing personal, health and therapy care for people across Sussex with acute and diverse needs. Your day will be varied and interesting, supporting people to achieve greater independence in their daily lives.


You willassistmulti-disciplinary colleagues in the ongoing assessment andprovision ofcare and reablementas prescribed within the personalised careplan.This includes simple clinical and therapeutic procedures such as taking physical observations, venepuncture,administering medications, simple wound care, rehabilitation programmes andequipment provision.You willobservechanges in peoples’ status and keep your team informed through verbal updates andaccuraterecord keeping.


Youdon'tneedpreviousexperience;we’lltrain you on the job, supporting you to be the best that you can be. Your training as aHealth andTherapy Assistant will include basic nursing and therapy skills andwe’llsupport you to achieve the Care Certificate.Through our comprehensive competencyframeworkwe will support you to achievecontinuous development.




You will need to able to drive with a full UK licence and access to a vehicle due to the geographic area covered by the team.

With experience and further training, you could further your career in health care opening opportunities to train as a Nursing Associate. With the appropriate qualifications and experience, you could also go on to train as one of the many degree-level healthcare professionals such as a Nurse, Podiatrist, Midwife or Therapist.
