As a prescribing clinical pharmacist, you will work alongside and be directed and supported by the current practice pharmacist, and will be an integral part of the team, providing a range of services and support to colleagues and patients, working to the following main areas: Telephone medicines support Medicine information to practice staff and patients Management of medicines at discharge from hospital Medicines Optimization Interface & Signposting Repeat Prescribing Risk stratification Information management Medicines quality improvement Medicines safety Implementation of local and national guidelines and formulary recommendations Education and training Care Quality Commission Public Health The job description will be reviewed and agreed with the successful applicant.
