We are recruiting for a Healthcare Assistant to join our Early Identification Service in Bradford. Salary: Marie Curie Clinical Pay Scale, aligned with AFC 2 £24,465 pro rata per annum Contract/Hours: Full Time and Part Time hours available Contract Type: Fixed Term 2 years Based: Bradford Closing date: 3rd September 2025 This service will be based in Primary Care within partnered GP surgeries in Bradford, where you will use the NHS early tool to search for patients who are not yet identified as having palliative care but are likely to be in the last year of life. You will be the initial contact with patients, offering support and the opportunity to complete advance care planning, and signposting to other services. You will visit patients in their own homes and refer them to either hospice, voluntary or community services. With experience of palliative care, you're ready and able to make a real difference. You will work in close collaboration and under the line management of the senior nurses in the service to provide direct support to patients and their families who are newly identified as having a palliative diagnosis. The role will include making first contact, arranging follow up and offering signposting to support services and advance care planning discussions so excellent communication skills are a must. The role will also involve visiting local community hubs to provide awareness, education and information on advance care planning. What were looking for: S/NVQ Level 2/3 qualification or an equivalent certificate/diploma is required Relevant community experience, providing care and support for people with life limiting illness or palliative care needs Advanced communication skills with the ability to have difficult and sensitive conversations.
