An exciting opportunity has arisen to lead the development of a progressive nursing care model within the PATCH team. This innovative service supports children with common paediatric conditions to receive high-quality care at home, reducing hospital admissions and improving continuity of care.
PATCH Aims:
• Deliver clinical systems to support early discharge and hospital-at-home care.
• Prevent re-attendances and admissions through strong community and GP links.
• Provide a 7-day service across NWL in collaboration with other PATCH teams.

• Use their experience and skills in the independent assessment and management of acutely unwell children to ensure all children referred to the PATCH team receive high quality, effective, individualised care that is compassionate and caring.
• Provide clinical supervision and clinical training for the band 6 members of the PATCH team and other nurses that rotate through the team.
• Work closely with the PATCH Matron, clinical leads and senior management teams to oversee clinical governance and audit of the PATCH service and to lead, monitor and feedback on the project performance.
• Assist in the reduction of acute admissions to inpatient hospital beds by supporting children and families in the community to self-manage conditions
• To participate in auditing effectiveness and incorporate findings into practice.
