Summary

To provide Clinical leadership and direction to the delivery of the Community Hospitals and Community Urgent Care (Physical Health) Directorate. Services include Inpatient beds, Outpatient services, Endoscopy, Community Theatres, Minor Injury and Illness Units, Countywide Rapid Response Service, The Hospital at Home team and the countywide Intravenous Therapy Service. To provide an element of clinical cover for Community Hospitals. To provide professional leadership and support to all working clinical colleagues within the Directorate. To professionally oversee the delivery of medical cover Service Level Agreements with GP partners into Community Hospitals alongside the Service Director. To advise colleagues of best evidence clinical practice in the field of Community Hospitals (inpatient, outpatient, endoscopy and theatres) and Community Urgent Care, with a specific focus on managing frailty, stroke rehabilitation and end of life care as well as admission and attendance avoidance opportunities. To enable a program of directorate medical teaching and support senior clinical leaders with the clinical supervision across the Community Hospitals, Minor Injury Units, Rapid Response and IV Therapies teams. To oversee the medical governance of the directorate alongside partners in the Nursing, Therapies and Quality Directorate, including reviews of incidents and attendance at learning and sharing events in line with the Patient Safety Incident Response Framework. This may include chairing some review meetings. To attend the Mortality Review Group and the System Mortality group and support reflection and learning, ensuring any specific points are reflected back to all medical practitioners and ACPs within the directorate. In a triumvirate with the Service Director and a nominated representative from the Nursing, Quality and Therapies directorate to oversee strategic decisions including system-wide urgent care transformation programme and neighbourhood health development. Work collaboratively with other services delivering urgent care such as Integrated Urgent Care Service to provide clinical leadership in clinical pathways developments. In a triumvirate with the Service Director and a nominated representative from the Nursing, Quality and Therapies directorate ensure colleagues working within the services deliver care in line with professional and regulatory compliance expectations. To assist the Trust in its relationship and networks with Commissioners and partner agencies within the ICS regarding clinical matters pertaining to all aspects of the services within the directorate. Specifically, to develop strong professional relationships with the local GP practices and Primary Care Networks, to support delivery of integrated care between primary and community services. To ensure that the organisations key values and principles are enacted within operational services. In conjunction with the Service Director to develop the skill mix within Community hospitals and community urgent care, including Advanced Clinical Practitioners and the consideration of other AHPs.
