Summary


TheeFraCCS and FrEDA Mobilisation Leadwill play a critical role within the Alliance Directorate, working across both Integrated Neighbourhood Development and the priority workstreams of Frailty and End of Life Care. This role will focus on driving the adoption and sustainable implementation of digital tools that enhance patient care, streamline clinical workflows, and improve data integration across health and care settings.



The postholder will work closely with system partners, frontline teams, and digital teams to ensure interoperability with, and onboarding to SystmOne, provide training and technical support, and increase awareness of digital solutions that support proactive and personalised care. Achievingconsistent uptake and use of SystmOne to support streamlined, effective, and equitable care delivery across all staff groups within the MSE footprint.

A key aspect of this role will be embedding digital innovations within Mid and South Essex Foundation Trust, ensuring alignment with strategic priorities and drive meaningful improvements in outcomes for individuals living with frailty or requiring palliative care.

The digital enablement will also need to facilitate the Transfer of Care Hub (TOCH) initiatives.

Include but are not limited to:
• Develop and maintain relationships with NHS organisations, primary care networks, social care providers, and community health teams.
• Act as the primary liaison between frontline professionals and digital development teams to ensure the tools meet clinical and operational needs.
• Advocate for the tool’s adoption at regional and organisational levels, aligning with healthcare transformation initiatives.
• Present at meetings, conferences, and professional networks to promote the benefits and impact of the tool.
• Work directly with frontline teams (clinicians, nurses, allied health professionals, social care staff) to support the practical implementation of the tool in daily workflows.
• Identify and address barriers to adoption of SystmOne, providing hands-on guidance and troubleshooting.
• Develop and deliver tailored support plans for different organisations to ensure long-term integration into routine practice.
• Hold RA SmartCard rights to enable the staff access and uptake.
• Design and deliver training sessions, webinars, and workshops to equip health and care professionals with the knowledge and confidence to use the tool effectively.
• Develop educational materials such as guides, FAQs, and video tutorials to support ongoing learning.
• Establish a network of "super-users" or champions within organisations to drive peer-led adoption.
• Work closely with IT and digital transformation teams to support the tool’s integration with existing systems and ensure interoperability.
• Provide first-line technical support and guidance, escalating complex issues to technical teams where necessary.
• Monitor system performance, user feedback, and technical challenges, ensuring continuous improvement.
• Track adoption rates, usage patterns, and the tool’s impact on clinical workflows and patient outcomes providing actionable insights to improve user adoption and user confidence.
• Collect qualitative and quantitative feedback from users to inform iterative improvements.
• Report on key performance indicators (KPIs) to leadership teams, commissioners, and funding bodies.
• Develop a long-term sustainability plan for the tool’s usage, embedding them into standard operating procedures and commissioning pathways.
• Identify opportunities for scaling adoption across wider health and care networks.
• Contribute to funding bids and business cases to support ongoing implementation and expansion.
