PART TIME PRACTICE NURSE Hours and Salary Negotiable Our successful, high achieving and innovative Surgery with a ANP and GP Joint Partnership, is offering you the opportunity to work with us. We are looking for an experienced Practice Nurse to join us working in a busy general practice as part of our dedicated clinical team along with friendly and supportive clinical and administrative staff. We have approximately 4600 patients. Main duties of the job The Post Holder will work as a member of the nursing team under the guidance of the Clinical Team to support the organisation and facilitation of clinical care in a holistic manner to patients registered at Westleigh Medical Practice. The Post Holder is a Practice Nurse who, acting within their professional boundaries within the NMC standards. You will perform routine tasks related to patient care as directed by the Clinicians; Cytology Health Checks, Health Promotion, ECGS, Asthma, Peak Flow Readings, Spirometry, Chronic disease management including diabetes, respiratory disease, CVD, Routine immunisations, childhood immunisations, chaperoning and assist patients where appropriate who are being examined by another clinicians. Actively participate in meeting national - QoF and locum contract targets, IIF, KLOE and DES. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Training Practice Gold/Silver Awards (University of Manchester) Clinical Duties Assist in and perform routine tasks related to patient care as directed by the clinical team. Cytology New Patient Health Checks/NHS Health Checks ECGs Asthma/Peak Flow Readings/Spirometry Venepuncture Urinalysis Chronic disease management including diabetes, respiratory disease, CVD Routine adult/childhood immunisations Chaperoning and assisting patients where appropriate who are being examined by another clinician. Requesting pathology tests, for example urine culture, swabs Family Planning for example annual pill checks, administration of Depo Injections. Following agreed clinical protocols with referral to senior clinicians or GPs as appropriate. Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Other Tasks will include: Clean and sterilise equipment Ordering of stock Ordering of vaccinations to maintain stock levels Participation in administrative systems in the Practice At all times there will be a need to maintain accurate records as per NMC guidelines. A duty to advise senior clinicians of potential problems or errors within the range of assigned tasks Attend and participate in all Practice Meetings when required To participate in developments within the practice and support QoF requirements.
