The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to patients across primary and community care settings. They will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of patients.
They will work closely with colleagues from the practice and wider service, to ensure that the practice operates in a safe, effective and responsive manner, achieving its performance targets. They will be responsible for helping the service to achieve its vision of providing competent, compassionate and sustainable care
The post holder will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
The post holder will support the development of General Practice Service Model in collaboration with multi-professional colleagues. They will play a crucial role in improving proactive management of patient care through timely assessment, referral, responsive intervention and anticipatory care planning.
Provide expert care and advice to a complex and dynamic caseload of patients presenting with a diverse range of medical conditions
Collaborate with other healthcare professionals to ensure comprehensive patient care
Maintain accurate and up-to-date patient records
Engage in continuous professional development and stay current with medical advancements and best practices
Undertakes first line comprehensive clinical assessment of patients, including those with complex presentations, employing an extended scope of practice including advanced consultation and physical assessment skills.
Review and initiates medicines through independent prescribing or Patient Group Directions
Our practice is based in the rural communities of Creswell and Langwith, situated on the borders of Nottinghamshire and Derbyshire. Along with our branch site Langwith Medical Centre, we serve a patient population of 5,776.
We are a training practice. We regularly host registrars and are committed to training and supporting upcoming doctors.
Our practices actively participate in research studies, and have achieved level 3 accreditation.
We are a team of:
· 5 General Practitioners
· 1 Advanced Clinical Practitioner
· 2 Practice Nurses
· 2 Health Care Assistants
Our Leadership Team includes Clinical Lead, Practice Manager, Deputy Practice Manager and Nurse Lead who are supported by a dedicated reception and administrative team. We also work collaboratively with the Hardwick and Bolsover Primary Care Network, as well as our service provider, Derbyshire Community Health Service FT, to provide the best possible community care for our patients.

Undertakes first line comprehensive clinical assessment of patients, including those with complex presentations, employing an extended scope of practice including advanced consultation and physical assessment skills.
Request, reviews and interpret diagnostic investigations within the context of other available information utilising a systematic process of clinical reasoning to formulate a differential diagnosis
Reviews and initiates medicines through independent prescribing or Patient Group Directions
Develops and records a treatment plan consistent with the outcome of assessment and the appropriate working diagnosis.
Recognises acutely unwell and/ or deteriorating patients, implements early interventions and escalates care where appropriate
Applies knowledge in palliative care to symptom control, recognition of dying and advance care planning
Maintains accurate and legible patient notes in accordance with the record keeping policy and professional guidance
To identify and report any safeguarding concerns or incidents and to act in accordance with safeguarding policies and procedures in order to safeguard patients
