Summary
Following assessment, where indicated, provide treatment using effective evidence-based interventions, as appropriate. Deliver one-to-one and group-based interventions. Ensure that information and progress regarding patients are communicated promptly and accurately to other team members and relevant organisations whilst having due regard for confidentiality. Where indicated act as MHP to ensure that each patient in their care receives an individual assessment and regularly updated care plan which is formulated with the service user. This will be in line with the Trusts Policy where indicated. Standard keyboard skills to be able to meet the IT requirements of the post Participate in regular clinical supervision with clinical supervisors and managerial supervision with line manager. Undertake relevant training and work towards accreditation where desired to develop in your role as SCM Worker.
