Summary
You should be motivated and be able to deliver high quality evidence based skilled nursing care within the community setting to meet the needs of the housebound patient. You will have good communication skills and be able to work autonomously but also work in an integrated way; as close working with our integrated rapid response teams, Community Matrons and social services colleagues is essential to ensure seamless care to out patient.
