Summary
The post holder will: Provide trauma informed practice to people in ED and follow up in the community in some cases. Liaise and work alongside other professionals to manage complex cases involving vulnerable children and young people. Maintain communication with the wider MDT when appropriate, these disciplines include Health and Social Care, Police, Education and Voluntary Sectors. Present, attend, influence, and develop relevant training in relation to these complex issues to ensure that the competency needs of healthcare staff and others are addressed. Advocate for the person to support them in working towards their personal goals. Liaise with other professionals to promote health and social wellbeing for individuals. Develop excellent working relationships with departments in the hospital, i.e., Paediatrics, Mental Health Liaison Team, etc. Support people and carers to access appropriate information and support, including sign posting to a range of support services. Experience of discussing ideas and assessments in a professional way, which supports the person in their best interests, communicating their thoughts and wishes. Maintain accurate electronic patient records and other data resources. Attend meetings and feedback as appropriate. Participate in data audit activity aimed at improving the quality of care. Participate in regular staff meetings. Flexibility to work within areas or departments to meet the demands of changing service needs. To support other colleagues when required in line with your competencies.
