Summary
Undertake a variety of duties as per the practice timetable, including face-to-face and telephone consultations, home visits, prescribing, and handling clinical correspondence in a timely manner. Provide professional, autonomous clinical decisions in relation to patient care, whether self-referred or referred by other healthcare professionals. Assess and manage patients with undifferentiated and undiagnosed problems, including screening for disease risk factors and early signs of illness. Develop care plans in line with current disease management protocols, involving patients in decisions about their care. Provide health promotion, education, and counselling as appropriate. Admit or discharge patients from the caseload and refer to other care providers when necessary. Maintain accurate, contemporaneous clinical records in accordance with practice standards. Compile and issue prescriptions using electronic systems and prescribe in line with the practice formulary. Support data collection and participate in clinical audits as required. Maintain the highest standards of clinical care and contribute to overall practice performance. Identify and escalate quality or safety issues to relevant team members. Take responsibility for clinical leadership and supervision of nursing and support staff within the team. Work effectively within the wider multidisciplinary team (MDT), supporting joint care plans and continuity of care. Explore opportunities for service modernisation, including preventative care initiatives and support for community-led or secondary care interface models. Provide expert advice to Trust or practice leadership on medical service development and strategic planning. Participate in the recruitment of clinical and support staff and assist HR in applying employment policies and procedures.
