Summary

The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources. The job holder will support the development of interdisciplinary and whole service working. As a role model they will promote the sharing of good practice and delivery of high quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers. The ICC service serves a population of approximately 3 million and is one of the largest in the UK. The ICC Service in Leeds is a well-established, multidisciplinary, regional service that provides specialist care for patients with 4 main groups of conditions: Cardiomyopathies - disorders of heart muscle Inherited arrhythmias - disorders of heart rhythm Aortopathies - disorders of the major blood vessels Neuromuscular conditions - disorders that affect skeletal muscle as well as the heart The successful applicant will support the existing team and contribute to the clinical care of patients with rare genetic diseases of the heart. They job holder will deliver nurse-led clinical services including the assessment of new patients and patients under long term follow up. They will be also involved in service development and patient education. They will also support the Senior Nurse Specialists in the delivery of the Sudden Cardiac Death pathway.
