The Band 4 Healthcare Assistant (HCA) plays a vital role within the Richmond Urgent Community Response (UCR) team, supporting the delivery of rapid, high-quality care to patients in their own homes during periods of acute illness or functional decline. Working under the guidance of registered professionals, the Band 4 HCA provides delegated clinical and personal care tasks, supports assessments, and contributes to care planning and delivery. The postholder will support timely interventions aimed at preventing hospital admission or facilitating early discharge, in line with the NHS UCR service model.
