Summary

Working independently and alongside GPs to identify appropriate care plans and treatment methods for patients through face to face appointments, telephone consultations and home visits Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in-depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor aliment services and prescription advice Acting as a Nursing lead to the Practice Nurse and HCA, ensuring they receive the relevant peer review, clinical supervision and support. This will be supported by the Lead Nurse of ELM Alliance To take a lead role on behalf of the practice areas of clinical function e.g. palliative, child protection etc Supporting the practice and PCN delivery of the Covid response / Vaccine delivery Acting on Path Lab results /investigation report and hospital correspondences. It is essential that the candidate has a track record of working as an Advanced Nurse Practitioner in a GP Practice or in other GP Primary Care Services.
